Instructions to complete the Application: This is a “fillable” PDF application. To complete the form:

1. Click the link for the application package (above). This will open the form in a new tab in your browser.
2. Download the form to your computer. Near the top right corner of the displayed form, you’ll see an icon that looks like a “down arrow” (the format may differ slightly depending on your browser). Click that icon to save the file.
3. Open the form using Adobe Acrobat.
4. Read the document entirely before filling in the application towards the end of the packet.
5. Complete the fields as necessary.
6. Save the edited file to your computer.
7. Launch an email message and upload the edited file as an attachment.

** PLEASE NOTE:If you are a NEW vendor, you must be juried by Midori Kai, Inc. prior to participation. Email Phyllis Osaki and Maureen Mukai at midorikaiboutique@gmail.com with your website link or photos of your product(s) and/or describe your food products. Midori Kai will jury your product(s) and inform you of your eligibility to participate in the 2025 Boutique.

Once approved, you can follow the steps outlined above to complete and submit the appropriate Application and payments.
